AAG Labs will collect information that identifies you ("personal information") only when you voluntarily provide it to us through our site or over the phone. In addition, we may collect information that you voluntarily provide to us through responses to questionnaires.

You have the right to look at or get copies of certain parts of your medical information. You may request that we provide copies in a format other than photocopies. We will use the format you request unless it is not practical for us to do so. You must make the request in writing. You may get the form to request access by using contact information listed at the beginning of this notice. If you request copies, we will charge you $5.00 for each page plus postage if you want them mailed to you. You may request that we communicate with you about your medical information by different means or to different locations. Your request to AAG Labs must be made in writing.
It is our policy to protect your account information against unauthorized access or release. All the information you provide to us is handled through a Secure Socket Layer ("SSL"). SSL is a leading Web technology that encrypts your account information. However, please be aware that e-mail is not secure.
In addition, we have procedures that limit AAG Labs' employees and contractors' access to personal information. Only those employees and contractors with a business reason to know have access to such information. We educate our employees about the importance of confidentiality and customer privacy through standard operating procedures, mandatory training programs, and internal policies on data privacy and corporate integrity. We take appropriate disciplinary measures to enforce employee privacy responsibilities.
